- Anstistolling afhankelijk van lokatie (mgl relatief)
- Reeds bestaand perifere zenuwletsel of – aandoening (relatief)
- 2e en 3e graads AV-blok
Complicatierisico
- Geen of onvoldoende werking blok
- Zenuwschade
- Incidentie (> 6 maand aanhoudend): van 0,14 tot 0,9 per 1000 1–3
- LAST (lokal anesthetic systemic toxicity)
- Incidentie: 0,08 tot 0,98 per 1000 1–4

- Hematoom
- Infecties

Dosisreductie
Overweeg dosisreductie bij de volgende groepen5:
- Patiënten >70 jaar: dosisreductie 10-20%
- Patiënten met een nierinsufficiëntie: dosisreductie 10-20%
- Patiënten met leverfunctiestoornissen:
- 1e block: normale dosis
- Herhaald block (< 5 halfwaardetijden) of continue toediening: dosisreductie 10-50%
- Patiënten met hartfalen:
- Licht/mild: geen dosisreductie
- Ernstig hartfalen óf herhaald block (<5 halfwaardetijden ) of continue toediening: dosisreductie 10-20%
- Hypoalbuminemie:
- Bij middelen met sterke eiwitbinding (levobupivacaine, ropivacaine): afhankelijk van mate van hypoalbuminemie lichte, matige of sterke dosisreductie toepassen
- Zwangeren:
- 1e trimester: géén of alleen kleine blocks met lage totale dosis
- 2e en 3e trimester: dosisreductie
- Neonaten en zuigelingen:
- Lagere toxische grens. < 6 maanden: dosisreductie 30%.
- Zieke kinderen:
- Lagere toxische grens: dosisreductie
Additieven (volwassenen)
- Adrenaline
- 2,5-5 ug/ml (1:400.000 – 1:200.000)
- Als testdosis bij detecteren van intravasculaire injecties
- Verlengen duur blok en verlagen van piekspiegels
- (Relatieve) contra-indicatie: bekende/ mogelijke schade aan zenuw of perineurale vascularisatie (diabeten of status na chemotherapie)
- Clonidine
- 0,5 -1 ug/kg toevoegen aan totaal toe te dienen volume
- verlenging blokduur met 2 – 2,5 uur
- Dexamethason
- 4 – 8mg toevoegen (perineuraal of IV)
- verlenging blokduur
